Our Assist-Life Stage, Transition Program

The program is designed specifically for individuals with special needs who are transitioning into adulthood. We provide support and guidance throughout this process, helping our clients to develop the skills they need to live independently. We also work closely with families to ensure that they are prepared for the changes ahead.
The program is flexible and tailored to each individual’s needs. We offer a variety of services, including: independent living skills training, career counseling, financial planning, and more. Our goal is to help our clients lead happy, successful lives.

- Development of independent self-care and living skills

- Financial mentoring
- Daily planning and Budgeting
- Establish supportive networks in a client’s community circle
- Linkage support to other supportive community services and programs
- Life planning and goal planning
- Assistance with accommodation and tenancy obligations
- Life transition planning including mentoring, peer support and individual skill development
- Assistance with decision making
